Below you will find a list of our policies and forms that will be sent to your email upon scheduling an appointment. You will need to read and fill them out prior to your scheduled treatment. 

If you can't find your intake paperwork in your inbox, you may want to check your spam or junk email folder. We recommend using a computer or tablet and a google chrome browser to complete the forms, and to further reduce the risk of glitches we recommend you do not use your phone. Please try to complete the forms in one sitting (it should only take 10-15 minutes) and submit when you are done.

To maintain our schedule and be respectful of all of our patient's time, you can confirm you scheduled appointment via email  or text and share a link to a secure site where you can access your New Patient Intake Paperwork. We appreciate that you complete all the paperwork, at home, ahead of your scheduled appointment. If you have any questions, please don't hesitate to reach out to the office for additional information.
